In order to qualify, you will need to meet the following criteria:

You must be morbidly obese. This is typically categorized by a BMI of 40 or higher or a BMI of no less than 35 with the presence of co-morbidities or you must be 100 pounds or more over your ideal weight.

You need to have tried to lose weight on your own unsuccessfully in the past. Many insurance companies will require records of participation regarding the methods you have tried.

You need to be willing and able to make a life change. Weight loss surgery can help you begin your journey, but it requires a lifelong commitment to exercising and changing your eating habits.
